Location Needed
- 1 position traveling within the Dept. of Behavioral Health Developmental Disabilities (DBHDD) regions to include the following counties: Cobb, Dekalb, Henry, Clayton
Seeking an experienced Support Coordinator with expertise in working with individuals with intellectual, developmental, or related disabilities. Responsibilities include planning and managing services to support individuals in living safe, healthy, and fulfilling lives at home and in the community. Duties also involve customer service, problem-solving, data entry, and administrative tasks.

Responsibilities
- Build and maintain relationships with individuals and stakeholders
- Conduct home and community site visits
- Facilitate service planning (ISP) and meetings
- Assist with provider selection, referrals, and monitoring services
- Track progress and share updates with stakeholders
- Keep accurate records of activities and progress
